Skizofrenia adalah salah satu bentuk gangguan kesehatan mental dengan permasalahan utama yang dialami pasien ketidakselarasan antara proses pikir, emosi dan kemauan. Skizofrenia merupakan gangguan mental yang masih sering mendapatkan stigma negatif dan diskriminasi dari lingkungan sekitar. Gejala yang muncul pada pasien skizofrenia biasanya akan mengalami penurunan atau kehilangan kemampuan menjalin hubungan dengan lingkungan sosial sekitar. Isolasi sosial ditandai dengan perasaan kesepian karena merasa ditolak, takut tidak diterima sehingga mempengaruhi keterampilan berkomunikasi dan bersosialisasi. Reaksi yang ditunjukkan orang yang mengalami isolasi sosial adalah perilaku menarik diri baik secara fisik maupun psikologis. Reaksi fisik ditunjukkan dengan individu menghindari sumber stressor. Dilakukan asuhan keperawatan dengan model studi kasus dengan pendekatan deskriptif analitik pada klien di RSJ Dr. Radjiman Wediodiningrat. Kesimpulan yang bisa ditarik dari hasil studi kasus ini adalah asuhan keperawatan yang tepat akan membantu menyelesaikan masalah klien dalam mengahadapi isolasi sosial menarik diri.
