Kajian Literatur tentang Pelayanan Farmasi Klinik dalam Pencegahan Medication Error

Abstract

Penelitian ini bertujuan untuk mengkaji peran pelayanan farmasi klinik dalam pencegahan medication error melalui analisis berbagai hasil penelitian yang relevan. Metode yang digunakan adalah penelitian kualitatif dengan pendekatan deskriptif melalui studi pustaka. Data diperoleh dari artikel ilmiah, systematic review, randomized controlled trial, observational study, dan berbagai dokumen akademik yang membahas medication error, medication reconciliation, drug related problems, dan pelayanan farmasi klinik. Teknik pengumpulan data dilakukan melalui penelusuran literatur dan analisis dokumen secara sistematis, sedangkan analisis data dilakukan melalui identifikasi tema, reduksi data, kategorisasi konsep, interpretasi temuan, dan penarikan kesimpulan secara induktif. Hasil kajian menunjukkan bahwa pelayanan farmasi klinik berperan penting dalam mencegah medication error melalui medication review, medication reconciliation, identifikasi drug related problems, optimalisasi dosis, edukasi pasien, serta kolaborasi multidisiplin. Intervensi farmasis klinis terbukti mampu menurunkan medication discrepancies, meningkatkan keamanan penggunaan obat, mengurangi adverse drug events, dan meningkatkan efisiensi biaya pelayanan kesehatan pada berbagai setting pelayanan, termasuk rawat inap, rawat jalan, intensive care unit, dan layanan onkologi. Selain itu, perkembangan virtual clinical pharmacy service juga memberikan kontribusi dalam meningkatkan akses pelayanan farmasi klinik di daerah dengan keterbatasan sumber daya. Kesimpulan penelitian ini menunjukkan bahwa pelayanan farmasi klinik merupakan komponen penting dalam sistem keselamatan pasien dan memiliki kontribusi signifikan dalam meningkatkan kualitas penggunaan obat serta mutu pelayanan kesehatan secara berkelanjutan.
